By using Care Companion you agree to be bound by this agreement. Please read every section before accepting.
1. License Grant
Care Companion, a subsidiary of Cre8ive AI LLC ("Company"), grants you a limited, non-exclusive, non-transferable, revocable license to access and use the Care Companion web application ("Application") for personal health management and professional caregiving activities.
2. Permitted Use
You may use the Application to:
- Track and manage medical appointments for clients under your home healthcare supervision
- Record appointment details including dates, times, providers, and clinical notes
- Generate and export appointment summaries for Medicaid caseworkers and authorized parties
- Manage mileage and caretaker attendance records for reimbursement documentation
3. Restrictions
You may NOT:
- Share your account credentials with any other person
- Use the Application for unlawful purposes or in violation of healthcare regulations
- Reproduce, distribute, modify, or create derivative works of the Application
- Reverse engineer, decompile, or disassemble any part of the Application
- Store Protected Health Information (PHI) under HIPAA without a separate Business Associate Agreement
- Resell, sublicense, or transfer your rights under this Agreement
4. Fees
The Application is currently provided without a subscription fee. The Company reserves the right to introduce paid subscription tiers or fees in the future. Any fees, billing terms, and renewal conditions will be clearly disclosed in advance and will require your acceptance before taking effect.
5. HIPAA Disclaimer
Care Companion is NOT a certified HIPAA-compliant platform. It is an organizational and scheduling tool. Users are solely responsible for ensuring compliance with all applicable federal and state healthcare privacy laws. Do not enter full Social Security numbers, complete medical diagnoses, or other sensitive Protected Health Information.
6. Intellectual Property
The Application and all content, features, and functionality are owned by Care Companion and its parent company Cre8ive AI LLC, protected by United States and international copyright and trademark laws.
7. Disclaimer of Warranties
THE APPLICATION IS PROVIDED "AS IS" WITHOUT WARRANTY OF ANY KIND. THE COMPANY DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE.
8. Limitation of Liability
IN NO EVENT SHALL CARE COMPANION OR CRE8IVE AI LLC BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, OR CONSEQUENTIAL DAMAGES. TOTAL LIABILITY SHALL NOT EXCEED THE AMOUNT PAID IN THE THREE MONTHS PRECEDING THE CLAIM.
9. Termination
This Agreement terminates automatically if you fail to comply with any term. Upon termination, you must cease all use of the Application immediately.
10. Governing Law
This Agreement is governed by the laws of the State of Nevada. Disputes shall be subject to the exclusive jurisdiction of courts in Clark County, Nevada.
11. Parent Company
Care Companion is a product developed and operated as a subsidiary of Cre8ive AI LLC. All rights and obligations are binding upon and inure to the benefit of Cre8ive AI LLC and its subsidiaries. Contact information will be available on the Care Companion website.

- Business Transfers: In connection with a merger, acquisition, or sale of assets
- With Your Consent: For any other purpose with your explicit consent
4. Data Storage & Security
Your data is stored in a secure cloud database (Supabase/PostgreSQL) with encryption in transit (TLS) and at rest, and per-user access controls enforced by Row Level Security. We implement commercially reasonable security measures. No method of electronic storage is 100% secure.
Your appointments, medications, and client data are securely stored in the cloud and will be available on any device when you sign in. A local cache is also kept on your device for offline access.
5. Data Retention
We retain account information while your account is active. If you delete your account or your data, account information is deleted or anonymized within 90 days, except where retention is required by law.
6. Your Rights
- Access: Request a copy of personal information we hold
- Correction: Request correction of inaccurate information
- Deletion: Request deletion of your personal information
- Portability: Request a portable copy of your data
- Opt-Out: Opt out of non-essential communications at any time
7. Children's Privacy
Care Companion is intended for adults (18+). We do not knowingly collect personal information from children under 13.
8. Third-Party Services
The Application may link to third-party services such as Google Maps. We are not responsible for the privacy practices of those services.
9. Changes to This Policy
We may update this policy at any time. We will notify you of material changes by posting the updated policy and updating the Effective Date. Continued use constitutes acceptance.

These Terms govern your access to and use of Care Companion, a product of Cre8ive AI LLC. By creating an account, subscribing, or accessing the Service, you agree to these Terms.
2. About the Service
Care Companion is a web-based application developed by Cre8ive AI LLC for individuals and caregivers to schedule and track medical appointments, manage medications, generate appointment documentation, and access mapping for appointment locations.
3. Eligibility
- Be at least 18 years of age
- Use the Service for your own personal health management, or as a caregiver, care coordinator, or home healthcare professional
- Have legal authority to enter into binding agreements
- Provide accurate and truthful registration information
4. Account Responsibilities
- Maintain confidentiality of your account credentials
- Accept responsibility for all activity under your account
- Immediately notify us of any unauthorized access or security breach
- Ensure use complies with all applicable laws and professional licensing regulations
One account is for one individual user. Sharing login credentials is prohibited and may result in immediate termination of access.
5. Fees
The Service is currently provided without a subscription fee. Cre8ive AI LLC reserves the right to introduce fees or paid subscription tiers in the future. Any fees, billing terms, cancellation rights, and refund policies will be clearly disclosed in advance, and continued use of any paid features will require your acceptance of those terms with at least 30 days written notice.
6. Professional Responsibility
Care Companion is an organizational tool — NOT a clinical decision support system, EHR, or HIPAA-certified platform. Users are solely responsible for compliance with HIPAA, state home healthcare regulations, and Medicaid program requirements.
- Do not use the Service as a substitute for professional clinical judgment
- Obtain all necessary client consents before entering client information
- You are responsible for the accuracy of information entered
- Comply with your employer's or agency's data handling policies
7. Acceptable Use
You agree NOT to:
- Violate any applicable law, regulation, or professional code of conduct
- Transmit viruses, malware, or other harmful code
- Attempt to gain unauthorized access to the Service or infrastructure
- Use the Service for any commercial purpose other than your own professional practice
8. Intellectual Property
All content, design, code, and materials within the Service are the exclusive property of Care Companion and Cre8ive AI LLC. You may not copy, modify, distribute, or create derivative works without prior written consent.
9. Indemnification
You agree to indemnify, defend, and hold harmless Care Companion and Cre8ive AI LLC from and against any claims, liabilities, damages, and expenses arising from your use of the Service or violation of these Terms.
10. Disclaimers
THE SERVICE IS PROVIDED "AS IS." CARE COMPANION AND CRE8IVE AI LLC DO NOT WARRANT THAT THE SERVICE WILL MEET YOUR REQUIREMENTS, BE UNINTERRUPTED, TIMELY, SECURE, OR ERROR-FREE.
11. Limitation of Liability
NEITHER CARE COMPANION NOR CRE8IVE AI LLC SHALL BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, PUNITIVE, OR CONSEQUENTIAL DAMAGES. TOTAL LIABILITY SHALL NOT EXCEED THE AMOUNT PAID IN THE THREE MONTHS PRIOR TO THE CLAIM.
12. Dispute Resolution
Disputes shall be addressed first through good-faith negotiation. If unresolved within 30 days, disputes will be resolved by binding arbitration in Clark County, Nevada, under the rules of the American Arbitration Association.
13. Governing Law
These Terms are governed by the laws of the State of Nevada. Legal actions shall be brought exclusively in courts of Clark County, Nevada.
14. Modifications
We may modify these Terms with 14 days notice via email or in-app notification. Continued use after the effective date constitutes acceptance.
15. Parent Company
Care Companion is a subsidiary of Cre8ive AI LLC. All rights, obligations, and liabilities are binding upon and inure to the benefit of Cre8ive AI LLC. Contact details are available on the Care Companion website.
16. Entire Agreement
These Terms, together with our Privacy Policy and End User License Agreement, constitute the entire agreement between you and Care Companion regarding your use of the Service.
